Just wanted to reach out and touch base before those of you with young adult children welcome some of them home for winter break.

This is a reminder that for those Legacy Program Clients who have young adult children 18-25 years old, we are happy to provide simple HIPAA Authorizations and Advanced Health Care Directives (AHCD) as part of the program.

Health Care Directive allows your young adult child to designate you or whoever they want as individuals who can help make medical decisions for them if they were unable to make it for themselves, if they were incapacitated.

The HIPAA Authorization would allow your child to give you the right to communicate with doctors about their private medical information, and to communicate with the school about an emergency situation in the event that they were not able to provide the authorization.

We will prepare these documents per YOUR instructions and then you can present them to your adult child(ren) to review and sign/notarize next time they are home.

e.g. for my child named Joseph Jones, set up Mom as the first health care agent and Dad as the second agent. Put Mom, Dad and brother Randy Jones as authorized parties under the HIPAA authorization.
