Wishing you a wonderful Summer and reaching out as we get ready for Fall. Some of you may be sending your young adults to college for the first time or returning there.

This is a reminder that for those Legacy Program Clients who have young adult children 18-25 years old, we are happy to provide simple HIPAA Authorizations and Advanced Health Care Directives (AHCD) as part of the program.

A Health Care Directive allows your young adult child to designate you or whoever they want as individuals who can help make medical decisions for them if they were unable to make it for themselves, if they were incapacitated.

The HIPAA Authorization would allow your child to give you the right to communicate with doctors about their private medical information, and to communicate with the school about an emergency situation in the event that they were not able to provide the authorization.

We will prepare per the parents’ instructions:

e.g. for my child named Joseph Jones, set up Mom as the first health care agent and Dad as the second agent.  Say they are open to organ donation. Put Mom, Dad and brother under HIPAA.

We will send the completed document to the parents for so they can be notarized/witnessed with the kids.
